Citizen Complaint Instructions and Form
If you have a complaint against an officer, please open read the instructions below before you fill out the complaint form. Just click the appropriate file to open it. You must print out the “Complaint Form” to your printer and then fill it out as it requires your signature.

Accident Participation Form
If you were involved in an accident that was not investigated and have been told to complete an “Accident Participation Form”, please click on this form below. Once opened, you can fill it out on the computer and then print it out. Please mail your completed form to the address on the form.

If you have been asked or need to get a proof of insurance form, click on the link below. You can fill the form out online, print it out, and then mail it to the address listed on the form.
